解毒活血汤内服联合复方双金痔疮膏治疗痔疮临床研究

摘要
目的:探讨解毒活血汤内服联合复方双金痔疮膏治疗痔疮的临床疗效及对血浆5-羟色胺水平的影响。方法:将120例患者采用随机数字表法分为观察组和对照组各60例。对照组采用双金痔疮膏,外敷后用无菌纱布覆盖,每天2次;观察组在对照组的基础上加用解毒活血汤内服,每天1剂。治疗前后观察患者疼痛改善情况、痔核表面黏膜改变情况、痔核大小变化情况及肿胀等主要症状、体征评分,检测治疗前、治疗后第3天、第7天血浆5-羟色胺水平,进行安全性评价。结果:观察组有效率为95.00%,对照组有效率为76.67%,两组比较差异有统计学意义(P<0.05);两组疼痛、坠胀、痔核大小及黏膜表面评分情况比较,治疗后均较治疗前显著改善(P<0.01),观察组较对照组改善更为显著(P<0.01);两组血浆5-羟色胺水平变化情况比较,两组均较治疗前呈明显下降趋势(P<0.05),治疗后第7天观察组显著低于对照组(P<0.01);两组治疗期间未发现明显与药物相关不良反应,治疗前后心、肝、肾功能未见明显异常。结论:解毒活血汤内服联合复方双金痔疮膏外敷治疗痔疮能明显改善患者的临床症状、体征,提高疗效,且安全性良好。
Objective: To investigate the clinical efficacy of Jiedu Huoxue Decoction combined with Compound Shuangjin Hemorrhoid Ointment in treatment of hemorrhoids and its effect on plasma 5-HT level. Methods: 120 patients were randomly divided into observation group and control group( with 60 cases in each group). The control group was treated with Compound Shuangjin Hemorrhoid Ointment,2 times a day and being covered with sterile gauze after treatment; the observation group was treated with Jiedu Huoxue Decoction in addition to that of the control group. Before and after treatment,the change of mucosal surface of hemorrhoids,the change of hemorrhoid size and the main symptoms and signs of hemorrhoid,the plasma levels of serotonin and the clinical security were measured before treatment as well as 3 days and 7 days after treatment. Results: The effective rate of the observation group was 95. 00% and that of the control group was 76. 67%. There was significant difference between the two groups( P <0. 05). The pain,bulge,hemorrhoid size and mucosal surface score of the two groups were significantly different after treatment( P < 0. 01) and the improvement of the observation was more obvious( P < 0. 01). The levels of plasma serotonin of the two groups were significantly lower than those before treatment( P < 0. 05). Seven days after treatment,the levels of plasma serotonin the observation group was significantly lower than that of the control group( P < 0. 01). There was no obvious drug-related adverse reaction in the observation group before and after treatment,and there was no significant abnormality was found in patients' heart,liver and kidney functions before and after treatment. Conclusion: Jiedu Huoxue Decoction combined with Compound Shuangjin Hemorrhoid Ointment in treatment of Hemorrhoids can significantly improve the clinical symptoms,signs,with better efficacy and safety.
