髋关节置换术中下肢深静脉血栓形成的因素分析与防护措施

摘要
目的探讨髋关节置换术中下肢深静脉血栓形成的影响因素,并提出相应防护措施。方法选取我院2016年2月至2018年2月收治的92例髋关节置换术患者作为研究对象,其中36例患者发生下肢深静脉血栓,56例患者未发生下肢深静脉血栓,采用单因素分析影响髋关节置换术下肢深静脉血栓形成的因素,并采用logistic回归分析法确定因素。结果经多因素logistic回归分析发现,年龄、病史、D-二聚体、手术时长为影响髋关节置换术中下肢深静脉血栓形成的重要因素。结论年龄、病史、D-二聚体、手术时长为导致髋关节置换术中发生下肢深静脉血栓的直接诱因,且并非独立存在,相互影响和作用,除提高护理人员与术者配合默契度外,加强防护至关重要。
Objective To explore the influencing factors and preventive measures for deep venous thrombosis of lower extremity during hip arthroplasty.Methods To select 92 cases of patients with hip arthroplasty who were treated in our hospital from February 2016 to February 2018 as the study subjects,among which 36 patients had deep venous thrombosis of lower extremities,and 56 patients didn't have deep venous thrombosis of the lower extremities. Factors associated with deep venous thrombosis of the lower extremity during hip arthroplasty were analyzed by univariate analysis. And logistic regression analysis were used to determine the factors. Results Multivariate logistic regression analysis revealed that age,medical history,D-dimer,and length of surgery were important factors influencing deep venous thrombosis of lower extremities during hip arthroplasty. Conclusions Age,medical history,D-dimer,and length of surgery are the direct causes of deep venous thrombosis of lower extremities during hip arthroplasty,and they are not independent risk factors.They have mutual influence and effect. In addition to improving the tacit understanding between nursing staff and surgeons,it is essential to strengthen protection.
