骨肽注射液联合丹参酮注射液对胫骨骨折术后血清sICAM-1、VEGF、TGF-β1、ALP水平及骨折愈合的影响

摘要
目的探究骨肽注射液联合丹参酮注射液对胫骨骨折术后血清相关指标及骨折愈合的影响。方法用计算机随机数字法将86例胫骨骨折患者分为观察组和对照组各43例。两组患者均接受切开复位内固定治疗,术后予以常规治疗,在此基础上,对照组患者予以骨肽注射液治疗,观察组患者予以骨肽注射液+丹参酮注射液治疗。比较两组患者骨折愈合程度和愈合时间差异,观察治疗前后血清指标[可溶性细胞间黏附分子-1(sICAM-1)、血管内皮生长因子(VEGF)、转化生长因子-β1(TGF-β1)、碱性磷酸酶(ALP)]、骨代谢指标[降钙素(CT)、骨钙素(BGP)、血清Ⅰ型前胶原羧基端肽(PICP)、Ⅰ型胶原羧基端肽β特殊序列(β-CTX)]、炎症因子[白细胞介素(IL)-2、IL-6、IL-8、肿瘤坏死因子-α(TNF-α)]等变化情况。结果观察组患者骨折愈合总有效率高于对照组,愈合时间短于对照组,差异有统计学意义(P<0.05);治疗4周后,两组患者血清sICAM-1、ALP、β-CTX、IL-6、IL-8、TNF-α水平均较治疗前降低,且观察组低于对照组(P<0.05),血清VEGF、TGF-β1、CT、BGP、PICP、IL-2水平均较治疗前升高,且观察组高于对照组(P<0.05)。结论胫骨骨折患者术后联合使用骨肽注射液和丹参酮注射液能调节血管和骨骼生长,有效促进骨折愈合,缩短康复时间,并可改善骨代谢水平,减轻炎性反应。
Objective To explore the effects of ossotide injection combined with tanshinone injection on serum related indexes and fracture healing after tibial fracture surgery.Methods 86 cases of patients with tibial fractures were divided into observation group and control group with 43 cases in each group according to the computer random number method.The two groups were treated with open reduction and internal fixation,and then given routine treatment after surgery.On this basis,control group was given ossotide injection,and observation group was given ossotide injection and tanshinone injection.The fracture healing degree and healing time were compared between the two groups,and the levels of serum markers[soluble intercellular adhesion molecule-1(sICAM-1),vascular endothelial growth factor(VEGF),transforming growth factor-β1(TGF-β1),alkaline phosphatase(ALP)],bone metabolic markers[calcitonin(CT),bone gla protein(BGP),serum procollagen type I cterminal peptid(PICP),type I collagen carboxy-terminal peptideβspecific sequence(β-CTX)]and inflammatory factors[interleukin(IL)-2,IL-6,IL-8,tumor necrosis factor-α(TNF-α)]were observed before and after treatment.Results The total effective rate of fracture healing in observation group was higher than that in control group,and the healing time was shorter than that in control group(P<0.05).After 4 weeks of treatment,serum levels of sICAM-1,ALP,β-CTX,IL-6,IL-8 and TNF-αin the two groups were lower than those before treatment,and the levels in observation group were lower than those in control group(P<0.05),and serum levels of VEGF,TGF-β1,CT,BGP,PEP and IL-2 were higher than those before treatment,and the levels in observation group were higher than those in control group(P<0.05).Conclusion Ossotide injection combined with tanshinone injection for patients after tibial fracture surgery can regulate blood vessel and bone growth,effectively promote fracture healing,shorten recovery time,and improve bone metabolism and reduce inflammatory reaction.
