摘要
目的:探讨甲状腺良恶性结节超声造影参数的差异及其与病灶内血管新生及细胞增殖的相关性。方法:选择在本院筛查出甲状腺结节的患者178例,经细针穿刺病理检查分为甲状腺良性结节组146例,甲状腺癌组32例。两组患者均进行超声造影并获取相关参数,采用荧光定量PCR法检测病灶组织中血管新生及细胞增殖相关基因表达量,采用Pearson检验评估甲状腺癌超声造影参数与恶性分子表达量的相关关系。结果:甲状腺癌组患者AUC、Peak的水平低于甲状腺良性结节组;血管新生相关基因HIF-1α、VEGF、TTF-1、MMP-9、COX-2mRNA的表达量高于甲状腺良性结节组;细胞增殖相关基因AG490、CCNG2、PP4R1、TBB mRNA的表达量低于甲状腺良性结节组,BRD4、TPX2 mRNA的表达量高于甲状腺良性结节组。Pearson检验发现,甲状腺癌患者的病灶超声造影参数AUC、Peak水平与血管新生、癌细胞增殖相关基因表达量均直接相关。结论:甲状腺癌病灶超声造影参数水平异常,具体异常程度与肿瘤血管新生、细胞增殖旺盛程度直接相关。
        Objective:To investigate the differences in ultrasonography parameters of benign and malignant thyroid nodules and their correlation with angiogenesis and cell proliferation within the lesions.Methods:178 patients with thyroid nodules who were screened in this hospital between August 2015 and October 2017 were divided into benign thyroid nodule group(n=146)and thyroid carcinoma group(n=32)according to pathological examination via fine needle aspiration.Two groups of patients received ultrasonography to obtain the relevant parameters,the fluorescence quantitative PCR method was used to detect the expression of genes related to angiogenesis and cell proliferation,and Pearson test was used to evaluate the correlation between thyroid cancer ultrasonography parameters and malignant molecule expression.Results:AUC and Peak levels in thyroid carcinoma group were lower than those in benign thyroid nodule group;angiogenesis-related genes HIF-1α,VEGF,TTF-1,MMP-9 and COX-2 mRNA expression were higher than those in benign thyroid nodule group;cell proliferation-related genes AG490,CCNG2,PP4 R1 and TBB mRNA expression were lower than those in benign thyroid nodule group whereas BRD4 and TPX2 mRNA expression were higher than those in benign thyroid nodule group.Pearson test showed that the lesion ultrasonography parameters AUC and Peak levels in patients with thyroid cancer were directly correlated with the expression of genes related to angiogenesis and cancer cell proliferation.Conclusion:The ultrasonography parameter levels of thyroid cancer lesion are abnormal,and the specific abnormal degree is directly correlated with tumor angiogenesis and cell proliferation.
