自创单人操作一钳法包皮环切术的临床应用

摘要
目的介绍笔者自创单人操作一钳法包皮环切术的手术方法与效果。方法自2007年10月至2015年10月应用自创单人操作一钳法包皮环切术治疗包皮过长、包茎760例。阴茎根部神经阻滞麻醉后,先用齿镊或止血钳在阴茎背侧冠状沟正中处夹一标记,后沿正中标记处及系带处上下两侧对称提起包皮,用一大弯钳从系带处自冠状沟正中标记处钳夹,将过长包皮自弯钳下方剪除,显露创面,结扎皮下出血点,内外板边缘用5-0可吸收羊肠线间断缝合,2周内5-0铬制可吸收羊肠线自行脱落,无需换药拆线。结果所有病例切缘整齐、美观、出血量少、术后恢复快、不易造成包皮切除过多或过少,无需用电刀止血,术者可独立完成操作。结论此术式特别适合基层医院医生,是一种值得推广的新型手术方法。
Objective: To introduce the technique and effect of self-created one-cut circumcision technique by clamp with single handler. Methods: A total of 760 patients with redundantprepuce or phimosis underwent circumcision by one-cut with clamp, after the penis nerve block anesthesia, to make a marker in the middle of penile dorsal coronary ditch with tooth tweezers orhemostatic forceps, then clipped up prepuce in bilateral symmetry, clamping with a large curved pliers from frenulum of prepuce since coronary groove in the middle of marker, to cut off redundantprepuce that below plier, showing the wound, ligation hypodermic haemorrhage,to suture inside and outside the prepuce edge with a 5-0 absorbable gut which could fall off in 2 weeks,did not need to takeout stitches. Results: All patients had tidy cut edge, beautiful appearance, less blood loss, postoperative recovery fast, without electricity knife hemostatic and performer could operate independently. Conclusion: This operation approach is especially suitable for grassroots hospital doctor which deserves wide clinical application.
