对比分析DHS、PFNA固定运用于股骨粗隆间骨折高龄患者治疗中的效果

摘要
目的:对DHS、PFNA固定运用于股骨粗隆间骨折高龄患者治疗中的效果进行对比研究。方法:将我院2014年7月到2016年7月诊疗的80例股骨粗隆间骨折高龄患者作为研究对象,根据手术实施方案不同,将患者分成DHS治疗组与PFNA治疗组,每组均40例患者,分析比较两组手术一般情况、)髋关节功能恢复程度、并发症出现情况。结果:DHS治疗组较PFNA治疗组手术时长显著延长,且手术出血量相对于PFNA治疗组明显较多,对比差异具有统计学意义(P<0.05);PFNA治疗组患者术后12个月、术后16个月髋关节功能分值均显著高于DHS治疗组,两组对比差异具有统计学意义(P<0.05);DHS治疗组治疗优良率为85.0%,PFNA治疗组92.5%,PFNA治疗组治疗优良率相比于DHS治疗组明显较高,差异具有统计学意义(P<0.05);DHS治疗组与PFNA治疗组并发症发生率分别为25.0%、10.0%,DHS治疗组并发症发生率相比PFNA治疗组上升明显,两组对比差异具有统计学意义(P<0.05)。结论DHS、PFNA固定运用于股骨粗隆间骨折高龄患者的治疗效果均十分显著,PFNA手术时长与手术出血情况均优于DHS,手术后患者并发症出现相对较少,患者髋关节好转较明显,临床治疗具有可靠性。
Objective:to DHS and PFNA fixed applied effect in the treatment of intertrochanteric fractures in senile patients.Methods:from July 2014 to July 2016 diagnosis and treatment of 80 cases of femoral intertrochanteric fractures in senile patients as the research object,according to different operation plan,the patients into the DHS group and PFNA treatment group,40 patients in each group were,analysis generally,compared two groups of surgery) the degree of hip joint function recovery,complications.Results the DHS treatment group PFNA treatment group had a significandy extend the operation time,blood loss and surgery compared with PFNA treatment group was obviously more,contrast differences statistically significant(P < 0.05);12 months postoperatively in patients with PFNA treatment group,16 months after hip function scores were significantly higher than that of DHS treatment group,compared the two groups statistically significant difference(P <0.05);DHS treatment group was 85.0%,PFNA treatment group was 92.5%,PFNA treatment group was significantly higher than DHS treatment group,the difference is statistically significant(P < 0.05);DHS group and PFNA treatment group complication rates were 25.0%,10.0%,the complication rate of DHS treatment group compared with PFNA treatment group increased significantly,compared the two groups statistically significant difference(P < 0.05).Conclusion:DHS and PFNA fixed used in the treatment of elderly patients with intertrochanteric fracture effect is very significant,PFNA operation time and operation bleeding were better than DHS,relatively few patients with complications after the surgery,patients with hip improved obviously,clinical treatment with reliability.
