非酒精性脂肪性肝病患者血清同型半胱氨酸浓度的意义

摘要
目的:分析非酒精性脂肪性肝病(NAFLD)患者不同血清同型半胱氨酸浓度的临床意义;方法:根据诊疗指南、磁共振结果并结合天冬氨酸转移酶(AST)、丙氨酸氨基转移酶(ALT)、谷氨酰转肽酶(GGT)将患者分为:非酒精性脂肪肝组(A组):25例,非酒精性脂肪肝炎(B组)19例,C组为正常对照者20例,空腹抽取静脉血,检查同型半胱氨酸(HCY)、总胆固醇(TC),三酰甘油(TG),AST,ALT,GGT、前白蛋白(PA)、空腹血糖(FBG)、空腹胰岛素(FINS)、肿瘤坏死因子α(TNF-α)、高敏C反应蛋白(hs-CRP)、脂联素(ADPN)、白细胞介素-6(IL-6)、血管生长因子(VEGF)、叶酸、维生素B12等并计算胰岛素抵抗指数(HOMA-IS)。结果:A、B两组的TC、TG、ALT、GGT、FINS、HOMA-IS、比较无差别(P>0.05),但明显高于C组(P<0.05);B组hs-CRP、AST、TNF-α、IL-6、VEG浓度高于A组(P<0.05),A组高于C组(P<0.05);A、C两组的PA浓度无明显差异(P>0.05),但均高于B组(P<0.05);C组APN浓度高于A组(P<0.05),A组高于B组(P<0.05);A组HCY明显高于B、C两组(P<0.05),B、C两组比较无区别。结论:血清HCY水平较低可以提示NASH患者肝脏损伤程度较重,血清HCY或许可以成为NAFLD的非侵入性诊断指标之一。
Objective:To analyze the clinical significance of serum homocysteine concentration in patients with nonalcoholic fatty liver disease(NAFLD);.Methods:According to the guidelines for diagnosis and treatment and the result of magnetic resonance and aspartate aminotransferase(AST),and alanine amino transferase(ALT) transfer,valley ammonia acyl turn peptidase(GGT) patients were divided into:Nonalcoholic Fatty Liver Group(group A):25 cases,19 cases of nonalcoholic steatohepatitis(group B),group C was normal control in 20 cases.Fasting venous blood,check the homocysteine(Hcy),total cholesterol(TC),triglyceride(TG),AST,alt,GGT,prealbumin(PA),fasting blood glucose(FBG),fasting insulin(fins),tumor necrosis factor alpha(TNF alpha),hypersensitive C-reactive protein(hs CRP) and adiponectin(ADPN),leukocyte mediated IL-6(IL-6),vascular growth factor(VEGF),folic acid,vitamin B12 and calculate the insulin resistance index(HOMA-IS).Results:Concentration of PA in group C were higher than that of A and B two groups of TC,TG,alt,GGT,fins,HOMA-IS and there is no difference(P>0.05),but was significantly higher than that of group C(P<0.05);B group,hs CRP,AST,TNF alpha,IL-6,VEGF concentration was higher than group A(P<0.05).In group A(P<0.05);a,C two groups had no significant difference(P>0.05),but was higher than that in group B(P<0.05);C group of APN concentrations higher than group A(P<0.05),a group was higher than that in group B(P<0.05);a group of Hcy was significantly higher than that in group B and C(P<0.05),and there was no difference in groupB and C.Conclusion:The lower level of serum HCY may indicate that the liver damage of NASH patients is heavy,and serum HCY may be one of the non invasive diagnostic indexes of NAFLD.
