焦虑抑郁状态对老年原发性高血压患者血压变异性的影响

摘要
目的:探讨焦虑抑郁状态对老年原发性高血压患者血压变异性的影响。方法:选取原发性高血压患者300例,均行SAS、SDS、HAMA、HAMD评分,行24h动态血压测量(ABPM),计算血压变异性(BPV)。结果:300例患者中,无焦虑、抑郁患者217例(72.33%),设为对照组;焦虑患者35例(11.67%),设为焦虑组;抑郁患者27例(9.00%),设为抑郁组;焦虑抑郁患者21例(7.00%)。焦虑组、抑郁组、焦虑抑郁组d SSD、d DSD、n SSD、n DSD、24h SSD、24h DSD均高于对照组(P<0.05或P<0.01);焦虑抑郁组d SSD、d DSD、n SSD、n DSD、24h SSD、24h DSD均高于焦虑组、抑郁组(P<0.05)。结论:焦虑抑郁状态在老年原发性高血压患者中较为常见,且增加了患者的血压变异性,是降压治疗的不利因素。因此应警惕老年原发性高血压患者可能存在的焦虑抑郁状态,对于合并焦虑抑郁状态的患者,在降压治疗时还应同步治疗焦虑抑郁。
Objective: To explore anxiety and depression's influence on blood pressure variability(BPV) of elderly patients with essential hypertension(EH). Methods: 300 elderlypatients with EH were selected, and were evaluated by SAS, SDS, HAMA, HAMD, and were measured by ABPM to calculate BPV.Results: In 300 elderly patients with EH, 217 cases(72.33%) without anxiety and depression was named as control group, 35 cases(11.67%) with anxiety was named as anxiety group, 27 cases(9.00%) with depression was named as depression group and 21 cases with anxiety and depression was named as anxiety and depression group. d SSD, d DSD, n SSD, n DSD, 24 h SSD, 24 h DSD in anxiety group, depression group, anxiety and depression groupwere all higher than that in control group(P < 0.05 or P < 0.01), and above indicators in anxiety and depression group were all higher than that in anxiety group and depression group(P < 0.05). Conclusion Anxiety and depression is common in elderly patients with EH, and it increases BPV's possibility, so it is negative fators for EH's treatment. We should be wary of anxiety anddepression in elderly patients with EH, and give synchronization treatment on anxiety and depression.
