盐酸关附甲素在急性缺血及慢性心功能异常动物模型中的血流动力学及电生理作用

摘要
盐酸夫附甲素(Guanfu base A,GFA)是一种C20二萜类结构的新型具有抗心律失常作用的生物碱,是从中草药黄花乌头【Aconitum coreanum (Levl)Raipaics】的块根中提取分离的有效成分。临床前试验已经证实,关附甲素在体内及体外可减慢心脏几乎所有部位的传导速度,且在不同的实验性心律失常动物模型中显示出抗心律失常作用。该药已经完成Ⅰ-Ⅲ期临床试验,并已经国家药品监督管理局批准正式上市,用于心律失常的急诊治疗。但目前完成的临床试验均是在没有或仅有轻度器质性心脏病患者中进行的,而急诊心律失常很多并发于心肌缺血和心功能异常,所以必须了解本药在这两种情况下的作用和安全性。据此本文设计了五个分实验:
     第一部分:盐酸关附甲素在急性缺血动物模型中的电生理学作用
     第二部分:盐酸关附甲素在急性缺血动物模型中的血流动力学作用
     第三部分:盐酸关附甲素在慢性心功能异常动物模型中的电生理学作用
     第四部分:盐酸关附甲素在慢性心功能异常动物模型中的血流动力学作用
     第五部分:盐酸关附甲素对动物模型心肌细胞的作用
     第一部分:盐酸关附甲素在急性缺血动物模型中的电生理学作用
     目的:研究分析盐酸关附甲素对结扎冠脉造成急性缺血的五指山小型猪的心电图及心内电生理指标影响。
     方法:选用成年五指山小型猪,雌雄不限,予静脉全麻后测量身长、体重,并行超声心动图检查。将实验动物随机分为结扎关附甲素(A)、非结扎关附甲素(B)、结扎生理盐水(C)三组,A、C组动物予开胸结扎冠脉前降支,B组暴露心脏后不予结扎。连接体表心电监测,在X线下经静脉送入三根电极导管,分别记录心房、房室交界区及心室电图。结扎至少30min后应用程控刺激方法测定药前心电
Hydrochloric Guanfu base A (GFA) is a new anti-arrhythmic alkaloid with a structure of C20-diterpenoid isolated from the tuber of Aconitum coreanum (Levl.) Raipaics. In pre-clinical studies it was shown to slow conduction velocity in nearly all compartments of the heart in vivo and in vitro and exhibited anti-arrhythmic effects in various experimental arrhythmia animal models. After finished phase I to III clinical trails, GFA has been approved by SFDA to treat some arrhythmias. Most of the participants in the clinical trials, however, were those with mild structure heart disease. This is not the case in emergency arrhythmias in which myocardial ischemia or cardiac insufficiency is complicated. It is necessary to evaluate the effect and safety of GFA in this setting. For this purpose the present study was designed with five parts:
    Part I: Electrophysiological Effects of Guanfu Base A on Acute Coronary Ischemic Models
    Part II: Hemodynamic Effects of Guanfu Base A on Acute Coronary Ischemic Models
    Part III: Electrophysiological Effects of Guanfu Base A on Chronic Cardiac Insufficiency Models
    Part IV: Hemodynamic Effects of Guanfu Base A on Chronic Cardiac Insufficiency Models
    Part V: The Effect of Guanfu Base A on myocardial cells of Animal
