通管方对输卵管炎性阻塞性不孕大鼠模型血清IL-1、TNF-α含量的影响

摘要
目的:通过观察通管方对输卵管炎性阻塞性不孕大鼠输卵管组织形态学改变及血清中白介素-1(IL-1)、肿瘤坏死因子α(TNF—α)含量的影响,来探讨其作用机制,以为临床治疗提供依据。
     方法:本实验以Wistar雌性大鼠为受试对象,用混合菌接种法注入输卵管复制炎性阻塞性模型,将180只雌性大鼠按随机排列表法抽样30只为正常组,除正常组外余150只按上述方法造模,造模20天后(除去因造模所致死亡的25只)剩余大鼠再按随即排列表法分为模型组24只、血府逐瘀片组25只、通管方高、中、低剂量组每组分别24只。在同条件下饲养,于造模第20天开始灌胃给药,其中高、中、低剂量组分别灌服高、中、低三种浓度通管方水煎剂,对照组灌服血府逐瘀片水溶剂,模型组及正常对照组正常饲养。每组又分为2个时间段,分别于开始服药至第15天、30天取材,然后采用HE染色观察输卵管组织形态学变化及ELISA法检测血清中IL-1和TNF-α含量。
     结果:1.用混合菌接种法注入输卵管复制其炎性阻塞性模型。于造模第5、10、15、20天取输卵管组织,经大体标本及形态学观察,证实模型复制成功。2.与模型组比较,通管方组和血府逐瘀片组能不同程度的对模型大鼠输卵管组织形态学进行改善。3.与模型组比较,通管方组和血府逐瘀片组能不同程度的降低模型大鼠血清中IL-1.TNF-α的含量,提高受孕率,P<0.01,差异有显著统计学意义;并且通管方的疗效优于血府逐瘀片。4.每组分别在治疗15天与30天后进行比较:①IL-1空白组和模型组在两个时间上差异无统计学意义,P>0.05;对照组和通管方组在两个时间上有差异有显著统计学意义,P<0.01;②TNF-α空白组、模型组及对照组在两个时间上差异无统计学意义,P>0.05;通管方组在两个时间上有差异有显著统计学意义,P<0.01。
     结论:1.用混合菌接种法注入输卵管复制输卵管炎性阻塞性模型是可行的。2.细胞因子:IL-1.TNF-α的改变与慢性输卵管炎性阻塞性不孕的发生发展有关。3.通管方可以改善输卵管炎性阻塞性不孕症大鼠输卵管结构和功能,对血清中IL-1.TNF-α的含量有调节作用从而改善其失衡状态,增强其抗病能力、自我修复能力,对大鼠炎性阻塞性不孕具有很好的治疗作用。4.通管方对血清中IL-1.TNF-α的改善作用,治疗30天后的作用比治疗15天后更好。
Objective:To probe the functional mechanism of the Tongguan soup and to instruct the cLinical treatment by studying the effect on the changes of the tubal morphology and cell factors(IL-1、TNF-α)of the rats with tubal inflammatory obstruction.
     Methods:According to random arrangement table method,30 Wistar female rats were allocated to normal group.Then the tubal inflammatory obstruction models were established by injecting the mixed bacteria.The left 150 Wistar female rats were randomly divided into 5 groups after being feeded for 20 days (except the 25 rats died after operation):model group (24 rats) controlled group (25 rats),high-dosage group (24 rats),medium-dosage group and low-dosage group with 24 rats. In the same conditions, the rats of the high-dosage group、medium-dosage group and low-dosage one were poured with various doses of Tongguan soup into stomach,the controlled group were treated with Xuefuzhuyu slice,the normal group and model group were both normal raise. Each group was divided into two time groups which were treated for 15 days and 30 days respectively before sacrifice.The pathologic identification of the tubal tissue were done by HE staining and the cytokines(IL-1、TNF-α) related to inflammation were determined in rats serum using ELISA kits.
     Results:1. The tubal inflammatory obstruction model was established by injecting the mixed bacteria.To confirm the model,the Pathologic identification of the tubal tissue were done by HE staining respectively after being model for 5、10、15、20days. The models were successfully established,which was confirmed by the results of HE staining and macroscopic observation.2. Compared with the model group,the controlled group and the groups by Tongguan soup can improve the tubal morphology to varying degrees.3. Compared with the model group,the controlled group and the groups by Tongguan soup can reduce the content of IL-1 and TNF-α,meanwhile increase pregnancy rates;Furthermore,the effects in the groups by Tongguan soup were superior to that in the group by Xuefuzhuyu slice, the difference between them was significant (P<0.01);4.Each groups were compared after being treated for 15d and 30d respectively:①There is no difference on the content of IL-1 between the normal group and the model group(P>0.05);There is difference on the group(P<0.01).②There is no difference on the content of TNF-αin every group (P>0.05)except the groups by Tongguan soup which have difference (P<0.01).
     ConcLusion:1.The tubal inflammatory obstruction model can be established successfully by injecting the mixed bacteria.2.The changes of the cytokines (IL-1、TNF-α) are reLated to the development of the infertility with Chronic oviduct inflammatory obstructive.3.The Tongguan soup can enhance the ability of disease-resistant and self-healing by regulating cytokines and improving the tubal structure and function of the rats with tubaL inflammatory obstruction,and therefore has good effects on treating the infertility with tubal inflammatory obstruction.4.The effect on the IL-1、TNF-αafter being treated by Tongguan soup for 30d is better than that for 15d.
