运动康复安全评定在胫骨平台骨折中的临床应用

摘要
目的:
     研究胫骨平台骨折术后通过骨科运动康复安全评定表制定个性化康复运动处方的疗效与安全性。
     方法:
     随访2003年1月~2007年12月胫骨平台骨折185例采取手术治疗的患者。其中对照组81例,术后采取固定,传统康复等常规治疗;康复组80例,根据骨科运动康复安全评定表制定个性化康复运动处方进行康复治疗。对其进行随访问卷调查,X线检查,并采用HSS膝关节功能评估系统分别评估术后6个月及12个月的膝关节功能。
     结果:
     术后随访时间6个月,12个月。
     ①按照HSS评分,康复组膝关节功能优良率术后6、12个月分别是93.8%和81.5% ,均显著高于对照组73.8%和66.2%(P     ②康复组术后6个月和12个月并发症发生率分别为9.9%,14.8%,对照组术后6个月和12个月并发症发生率分别为16.1%,35.2%。
     ③两组患者术后12个月内固定失败情况比较,康复组失败率为12.3%,对照组失败率为16.2%。
     结论:
     1、采用个性化运动康复处方对改善膝关节功能是有效的。
     2、根据骨折类型、固定程度、软组织情况三因素评分指导术后康复训练是安全的。
     3、骨折运动疗法安全性评定表为早期安全的运动康复治疗提供了参考依据,能有效地改善骨折患者的预后疗效,适合临床骨科的广泛应用。
Purpose:research tibial fractures postoperative rehabilitation through orthopaedic movement in formulating the safety assessment table personalized efficacy of rehabilitation exercise prescription;
     Methods:Made a follow-up visit in the January, 2003 ~2007 year in December shinbone platform bone fracture 185 examples to adopt the patient who the surgery treated. And control group 81 examples, after the technique, adopts fixedly, conventional treatments and so on traditional recovery; The recovery group 80 examples, evaluate the table formulation personalization recovery exercise prescription according to orthopedics movement recovery to carry on the recovery treatment safely. Carries on the revisit questionnaire survey to it, X inspection, and uses the HSS knee joint function appraisal system to appraise the technique latter 6 months and 12 month-long knee joint function separately.
     Results:We were followed up for 1 year.
     1 in accordance with the HSS score, excellent rate of knee rehabilitation group after 6.12 months were 93.8% and 81.5%, were significantly higher than 73.8% and 66.2% (P      2 rehabilitation group after 6 months and 12 months, complications were 9.9%, 14.8% in the control group after 6 months and 12 months, the incidence of complications was 16.1%, 35.2%.
     3 In two group of patient technique latter 12 months the fixed defeat situation comparison, the recovery group failure rate is 12.3%, the control group failure rate is 16.2%.
     Conclusion:
     1.using individualized exercise prescription to improve knee function recovery is effective.
     2.according to fracture type, fixed level, the three factors of soft tissue condition score guide postoperative rehabilitation training is safe.
     3.fracture physical therapy safety assessment form for the safe movement of the early to provide a reference for rehabilitation, can improve the prognosis of patients with fracture efficacy for a wide range of orthopedics applications.
