新源县囊型包虫病的流行病学调查

摘要
目的:通过横断面研究的方法对新源县的囊型包虫病流行现状进行调查,揭示囊型包虫病的分布特点和患病情况,探讨当地囊型包虫病流行的危险因素,为当地卫生行政部门制定囊型包虫病防治规划和采取综合防治干预的实施提供科学依据。方法:采用整群随机抽样的方法,对新源县的那拉提镇和吐尔根乡的常住居民进行血清学和B超检查,以及问卷调查。对感染囊型包虫病的危险因素进行单因素与多因素logistic分析。结果:新源县那拉提镇和吐尔根乡囊型包虫病的血清阳性率为25.28%,患病率为1.52%。那拉提镇血清阳性率19.15%,吐尔根乡31.35%,不同乡镇、性别和民族间血清阳性率间差异有统计学意义。不同职业患病率间差异有统计学意义。单因素的logistic回归分析结果显示,感染囊型包虫病的危险因素有地区、性别、年龄和民族,多因素分析结果显示不同地区、性别、年龄、族别和职业是危险因素。结论:新源县农牧区囊型包虫病的血清阳性率和患病率较高,血清阳性率女性高于男性,哈萨克族高于其他民族;患病率农牧民高于其他职业人群。今后应根据当地的流行现状加强农牧区囊型包虫病的防治工作,控制囊型包虫病的流行。
Objective: This study investigated the prevalence of human cystic echinococcosis (HCE) in Xinyuan County with method of cross-sectional study. The epidemic features such as distribution and human prevalence would be analyzed and the risk factors related with HCE would be discussed. Method: Random clustered sample was used in this study. An epidemiological survey with a preffessional questionnaire, serological test and ultrasound was carried out among permanent residents in Nalati Town and Tuergen Commune. The risk factors related with HCE was analzed by single factor and multi-factors logistic regression analysis. Results: The HCE seropositive and prevalence was 25.28%, 1.52% in Xinyuan county. Seropositive rates were 19.15% and 31.35% in Nalati Town and Tuergen Commune. Statistical significance was found in different areas, genders and ethnic groups. The prevalence of HCE had significance in different occupation. Logistic analysis of single factor showed the risk factors related with HCE were areas, genders, ages and ethic groups. The results of multi-factors logistic analysis indicated that areas, genders, ages, ethic groups and occupations were the risk factors related with HCE. Conclusions: The seropositive and prevalence of HCE occurs commonly in XinYuan. The seropositive rate of female and Kazak ethnic group were higher than other groups. The prevalence of farmers and herders were higher than other occupations. The prevention and control programme of HCE should be regarded and control in Xinyuan county for the future.
