住院医师规范化培训存在的问题及对策研究

摘要
目的:从规培学员视角探讨住院医师规范化培训运行过程中存在的问题以及解决的方法与对策。方法:采用调查问卷形式,以2015级、2016级规培学员为调查对象,调查本科生、研究生(已毕业)、专业学位研究生(双轨合一在读生)三类,发放问卷400份,回收问卷345份,回收率86.25%。对问卷中的问题进行分析,从中发现规培中的问题,并分析问题找出对策。结果:规培学员在薪资待遇、带教老师水平、规培质量、考核体系等方面效果不满意,三类规培学员在自我评价中,医患沟通能力、临床思维能力及业务综合能力方面差异有统计学意义(P<0.05),而规培中的专业外语能力及科研能力,均评价较低,研究生较本科生收获均较大,专业学位研究生在专业外语能力上术前两组比较,差异有统计学意义(P<0.05)。结论:临床住院医师规范化培训是一条漫长的探索之路,医学改革之路有很多问题需要去解决,只有在探索中发现问题才能解决问题。
        Objective: To investigate the current problems of and countermeasures against the standardized training of resident physicians. Methods: Questionnaires were performed in the students enrolled in 2015 and 2016. The subjects undergoing standardized residential training included undergraduates,graduates and graduate candidates( professional degree in clinical medicine). Of the total 400 copies of questionnaires,345 were retrieved( 86. 25%) and analyzed for the problems in current training practice. Results: The trainees were generally dissatisfied with their monthly income,expertise level of the supervisors,training quality and current appraisal mechanisms. The three groups of subjects surveyed were statistically different in the ability of patient-doctor communication,clinical thinking and comprehensive professional qualification by self-evaluation( P < 0. 05),and generally considered poor in command of the foreign language in medicine and competency in academic research. Graduates and graduate candidates thought that they learned more than the undergraduates,and the command degree of foreign language in medicine was different between graduates and graduate candidates in professional degree in clinical medicine( P < 0. 05). Conclusion: Standardized training of the resident physicians has a long way to go,and some problems need urge solving in reforming of current medical education.
