《针灸大成》眼病选穴规律探析

摘要
目的总结、分析《针灸大成》治疗眼病的针灸选穴及其组方规律,以期为临床针灸治疗眼病提供选穴参考。方法以《针灸大成》一书中所载治疗眼病的针灸选穴及处方为研究对象,整理归纳其用穴数目、常用穴位、穴位归经、穴位分布及特定穴应用,采用描述性统计方法对以上资料进行频次分析。结果纳入针灸处方共118个,穴位162个,累计用穴频次共444次。选穴数目1～2穴的处方最多,频次高达76次,频率为64.4%;穴位使用频次前十的穴位依次为合谷、攒竹、睛明、头临泣、太阳、肝俞、风池、丝竹空、小骨空、上星;经脉应用频次最多的为足太阳膀胱经,用穴频次为95次,占腧穴总频次的21.4%;头面部的腧穴数量及运用频次均居首位,共计54个穴位,使用频次达194次,频率为43.7%;治疗眼病的162个穴位中,属于特定穴的有109个,频率为67.3%,其中交会穴的使用频次最多,为117次,频率为26.4%。结论《针灸大成》治疗眼病,用穴少而精;多取阳经腧穴;选穴多归膀胱经;以局部选穴与循经选穴为主;多用特定穴。
Objective To summarize and analyze the acupoint selection and prescription rules for acupuncturemoxibustion treatment of oculopathy in Compendium of Acupuncture and Moxibustion, for providing reference for acupuncture-moxibustion treatment of oculopathy in clinical practice. Method The acupuncture-moxibustion acupoint selections and prescriptions for oculopathy stated in Compendium of Acupuncture and Moxibustion were taken as the study target. The number of the selected acupoints, commonly used acupoints, meridian classification, acupoint distribution and the use of specific acupoints were sorted, and frequency analyses of these data were made by descriptive statistical method. Result There were a total of 118 acupuncture-moxibustion prescriptions, involving 162 acupoints with a total frequency of 444. The prescriptions containing 1-2 acupoints were predominant and the frequency was 76, occupying 64.4%; the top ten acupoints on the list of frequency were Hegu (LI4), Cuanzhu (BL2), Jingming (BL1), Toulinqi (GB15), Taiyang (EX-HN5), Ganshu (BL18), Fengchi (GB20), Sizhukong (TE23), Xiaogukong (EX-UE6) and Shangxing (GV23); the most commonly used meridian was the Bladder Meridian of Foot Taiyang, and the frequency was 95, occupying 21.4%; acupoints located on the head and face were mostly selected and most commonly used, involving 54 points with a frequency of 194, occupying 43.7%. Among the 162 acupoints for oculopathy, 109 were specific points, accounting for 67.3%; the crossing points had the highest frequency, which was 117, occupying 26.4%. Conclusion The acupoint selection was concise in Compendium of Acupuncture and Moxibustion for oculopathy; acupoints from yang meridians were more commonly used; most of the acupoints were from the Bladder Meridian; acupoints were predominantly selected from the local area and along meridians; specific acupoints were often used.
