摘要

目的对静脉用药调配中心常见差错种类及原因进行分析、总结,探讨其防范措施及效果。方法对我院2017年11月—2018年3月静脉用药调配中心出现的差错记录资料进行回顾性分析,对差错种类、环节进行统计、分类,分析其发生原因。在2018年4—8月实施针对性防范措施,观察静脉用药调配中心防范前后差错发生率。结果静脉用药调配中心差错发生环节主要为摆药、调配、贴签、审方、核对、发放环节,防范后各环节差错发生率均低于防范前,差异具有统计学意义(P <0.05);防范前后差错发生率分别为0.44%、0.20%,差异具有统计学意义(P <0.05)。结论针对静脉用药调配中心常见差错的种类、原因进行分析,从而制定针对性防范措施,能够减少差错发生的风险因素,降低差错发生率,提高成品输液的质量及静脉用药的安全性。
        Objective To analyze and summarize the types and causes of common errors in intravenous drug dispensing center, and to explore the preventive measures and effects. Methods A retrospective analysis was made on the errors recorded in the intravenous drug dispensing center of our hospital from November 2017 to March 2018. The types and links of errors were counted and classified, and the causes were analyzed. Targeted preventive measures were implemented from April to August 2018 to observe the incidence of errors before and after prevention in intravenous drug dispensing centers. Results The main errors in intravenous drug dispensing center are dispensing, dispensing, labeling, reviewing, checking and issuing. The incidence of errors in each link after prevention is lower than that before prevention, and the difference is statistically significant(P< 0.05). The incidence of errors before and after prevention were 0.44%and 0.20% respectively, with statistical significance(P < 0.05). Conclusion This paper analyzes the types and causes of common errors in intravenous drug dispensing center, so as to formulate corresponding preventive measures, which can reduce the risk factors of errors, reduce the incidence of errors, improve the quality of finished product infusion and the safety of intravenous drug use.
