江西省肿瘤登记地区鼻咽癌疾病负担分析

摘要
目的对2009-2016年江西省肿瘤登记地区鼻咽癌报告情况进行分析和对比,评价其疾病负担。方法以2009-2016年江西省12个国家级肿瘤登记点鼻咽癌监测数据为基础,计算鼻咽癌的发病率和死亡率;同时,利用WHO提供的疾病负担分析模块计算死亡损失生命年(years of life lost due to mortality,YLL)、伤残损失生命年(years lost due to disability,YLD)、伤残调整寿命年(disability-adjusted life years,DALY)等指标。结果 2009-2016年江西省肿瘤登记地区鼻咽癌发病率为5.75/10万、死亡率为2.31/10万,男性发病率、死亡率高于女性,农村发病率、死亡率高于城市。鼻咽癌所致DALY为9352.23人年,其中YLL为9058.82人年,YLL所占比例高至96.86%;DALY率为0.39,男性DALY率高于女性,农村DALY率高于城市,60～69岁年龄组负担最重。结论江西省鼻咽癌疾病负担主要由早死所致,防治工作中应重点关注农村地区,尤其是45岁及以上男性人群;此外,应继续扩大江西省肿瘤登记人群的覆盖范围,健全恶性肿瘤监测系统。
Objective Analyzing and comparing the reporting situation ofnasopharyngeal cancer in cancer registries of Jiangxi Province from 2009 to 2016,in order to evaluating the disease burden ofnasopharyngeal cancer.Methods Based on the data of nasopharyngeal cancer in 12 national cancer registries in Jiangxi Province from 2009 to 2016,the incidence and mortality were estimated.Meanwhile the disease burden analysis module provided by WHO was used to calculate these indicators including years of life lost due to mortality(YLL),years lost due to disability(YLD)and disability-adjusted life years(DALY),etc.Results The incidence of nasopharyngeal cancer in cancer registries of Jiangxi Province from 2009 to 2016 was 5.75/100 000 and the mortality was 2.31/100 000.The incidence and mortality were both higher in male than female,and in rural area than in city.The DALY by nasopharyngeal cancer were 9352.23 person-years,among them,YLL were 9058.82 person-years and its proportion was up to 96.86%.The rate of DALY was 0.39,the male was higher than the female and the rural area was higher than the city;the disease burden of the 60～69 years old was the heaviest.Conclusion The disease burden of nasopharyngeal cancer in Jiangxi was caused by early death.The rural areas should be paid more attention in prevention and control programme,especially those males who were 45 years and older.In addition,the coverage should continue to be enlarged the population of cancer registration in Jiangxi Provinceand the cancer monitoring system should be improved.
