摘要

目的:比较用不同剂量的糖皮质激素治疗过敏性休克的效果。方法:将2016年5月至2018年2月期间蒲江县中医医院收治的96例过敏性休克患者平均分为A组、B组、C组。这些患者入院后,为其使用不同剂量的氢化可的松进行治疗。A组患者氢化可的松的用量为300 mg/次,B组患者氢化可的松的用量为200 mg/次,C组患者氢化可的松的用量是100 mg/次。治疗后,比较两组患者的临床疗效、各项生命体征恢复正常的时间及不良反应的发生率。结果:治疗后,A组患者的意识、呼吸的频率、血压、心率恢复正常的时间及全身皮疹消退的时间均短于B组患者和C组患者(P <0.01),其治疗的总有效率高于B组患者和C组患者(P <0.01);B组患者的意识、呼吸的频率、血压、心率恢复正常的时间及全身皮疹消退的时间均短于C组患者(P <0.01),其治疗的总有效率高于C组患者(P <0.01)。三组患者不良反应的发生率相比,P>0.05。结论:与每次使用100 mg、200 mg的氢化可的松治疗过敏性休克相比,每次使用300 mg的氢化可的松治疗过敏性休克的效果更为理想。
        Objective: To compare the effects of different doses of glucocorticoid in the treatment of anaphylactic shock. Methods: 96 patients with anaphylactic shock admitted to pujiang county hospital of traditional Chinese medicine from May 2016 to February 2018 were divided into group A, group B and group C on average.These patients were treated with different doses of hydrocortisone after admission. The dosage of hydrocortisone in group A was 300 mg/time, that in group B was 200 mg/time, and that in group C was 100 mg/time. After treatment, the clinical efficacy, the recovery time of vital signs and the incidence of adverse reactions were compared between the two groups. Results: after treatment, patients in group A had shorter recovery time of consciousness, breathing frequency, blood pressure, heart rate and systemic rash than those in group B and C(P < 0.01), and the total effective rate was higher than those in group B and C(P < 0.01). Patients in group B had shorter recovery time of consciousness, breathing frequency, blood pressure, heart rate and systemic rash than patients in group C(P < 0.01), and the total effective rate of treatment was higher than patients in group C(P < 0.01). Compared with the three groups, the incidence of adverse reactions was P > 0.05. Conclusion: compared with the treatment of anaphylactic shock with 100 mg and 200 mg hydrocortisone, the treatment of anaphylactic shock with 300 mg hydrocortisone is more effective.
