多排螺旋CT检查对乏脂肪肾血管平滑肌脂肪瘤的诊断价值

摘要

目的分析多层螺旋CT对乏脂肪肾血管平滑肌瘤的临床诊断价值。方法收集临床怀疑为乏脂肪肾血管平滑肌瘤的患者32例,均使用多排螺旋CT进行诊断,并对所有研究对象使用病理组织检查确诊,之后对诊断的一般情况进行分析,对乏脂肪肾血管平滑肌瘤患者多层螺旋CT的影像学表现和其对本病的诊断价值进行分析。结果 32例疑似患者中通过病理组织检查诊断为乏脂肪肾血管平滑肌瘤的患者25例,而通过多层螺旋CT诊断的患者21例,CT平扫主要表现为高密度,增强扫描肿块的强化均匀,且以延迟强化为主,且多层螺旋CT对乏脂肪RAML患者诊断的灵敏度为76. 0%,特异度为71. 4%,正确指数为47. 4%,ROC曲线下面积为0. 737。结论乏脂肪肾血管平滑肌瘤患者采用多层螺旋CT诊断时的灵敏度和特异度较高,误诊率和漏诊率较低,推荐在临床上应用。
        Objective To observe the diagnostic value of Multi-detector CT examination for renal angiomyolipoma with minimal fat. Methods 32 patients who were suspected renal angiomyolipoma with minimal fat were collected,all cases were diagnosed by multi-slice spiral CT,and all cases were confirmed by histopathological examination,then the general condition of the diagnosis was analyzed,and the imaging findings and diagnostic value of multi-slice spiral CT in renal angiomyolipoma with minimal fat were analyzed. Results Among the 32 suspected patients,25 were diagnosed as renal angiomyolipoma with minimal fat by pathological examination,and 21 cases were diagnosed by multislice spiral CT,CT scan mainly showed high density,enhanced scan showed homogeneous enhancement and delayed enhancement,the sensitivity of multi-slice spiral CT in diagnosing renal angiomyolipoma with minimal fat was 76. 0%,the specificity was 71. 4%,the correct index was 47. 4%,and the area under the ROC curve was 0. 737. Conclusion The sensitivity and specificity of multi-slice spiral CT is high in in renal angiomyolipoma with minimal fat patients,the rate of misdiagnosis and missed diagnosis is low,recommending to be applied in clinic.
