The goal for reconstruction following oncologic surgery involves providing adequate coverage allowing subsequent adjuvant therapy, preserving function, and to have a reasonable aesthetic outcome. Patients with soft tissue tumors of the lower extremity have en bloc tumor resections with wide margins for local control followed by reconstruction. This reconstructive algorithm demonstrates survival rates equivalent to amputation, with success approaching 95%. Attempting limb salvage has now become the mainstay standard for reconstructing defects following excision of lower extremity cancers. J. Surg. Oncol. 2016;113:955–961.
