Crowned dens syndrome is a rare pathology characterized by acute attacks of neck pain with fever, rigidity, general signs of inflammation and calcification of the periodontoid articular structures. Symptoms of crowned dens syndrome can mimic other disease, particularly meningitis. Consequently, this syndrome is often misdiagnosed, which leads to unnecessary invasive examinations, inappropriate treatment and prolonged hospitalization. In the present case report, we described two cases initially diagnosed as meningitis. Both patients had nuchal rigidity and neck pain without Kernig's sign and Lasegue sign. Discrepancy of meningeal irritation signs is the key point for diagnosis.
