Atrial flutter with 1:1 ventricular response is usually a condition associated with the use of antiarrhythmic drugs that achieve slow the flutter cycle length, allowing conduction to the ventricle. We present a short series of cases in which the 1:1 flutter occurred in the absence of antiarrhythmic drugs.
