This article reviews the use of transcatheter ablation for the treatment of atrial fibrillation. The eight published controlled studies comparing ablation with antiarrhythmic drugs have demonstrated that ablation is superior. In paroxysmal atrial fibrillation, circumferential pulmonary vein ablation with demonstrated electrical isolation is the preferred technique, and has a 75 % success rate, though more than one procedure is frequently required. In persistent atrial fibrillation, repeated extensive procedures are needed more often, but the final success rate is acceptable. This article also considers the complications reported. In addition, we summarize our findings in 197 patients: the complication rate was 2.5 % and effectiveness was similar to that reported in published series. Over a mean follow-up of 25 months, we observed that 93 % of patients experienced a significant improvement compared with baseline, before ablation. We propose indications for the procedure based on clinical practice guidelines.
