Prenatal diagnosis of fetal cardiac abnormalities detected by transvaginal echocardiography in early gestation

This study evaluate de accuracy of early fetal transvaginal echocardiography (TVE) in the detection of cardiac defects in fetuses at high risk for this condition. In our clinic we perform ultrasound measurement of fetal nuchal translucency thickness (NT), between 10+3 to 14+0 weeks gestation as a screening for fetal chromosomal abnormalities. As a pathological association between increased NT and cardiac defects has been previously described, we perform a detail fetal heart examination (by a specialist in fetal echocardiography, Dr. LML) in all cases with increased NT (> 2.5mm). Serial TVE was performed in 63 fetuses (from the early gestation until obtain a good evaluation of all cardiac structures). From these 63 fetuses, 50 had a normal NT and 13 had an increased NT. 14+2 weeks was the medium gestational age that a good view of the cardiac structures was obtained. In the group with normal NT we did not find any structural abnormalities. In the group with increased NT 4 cardiac abnormalities were observed: 2 hypoplastic left ventricle; 1 double outlet of right ventricle; 1 golf ball. Our findings show that despite of the limitations of early fetal cardiac evaluation it is possible to detect cardiac abnormalities at this stage of pregnancy and like previously described increased NT seems to be associated with cardiac defects.
