Introduction and Methods The authors review the clinical course, treatment and outcome of nine patients (7 females and 2 males, mean age 30 years) with symptomatic cavernous malformations of the thalamus. Four patients presented acutely with cerebral hemorrhage, two with progressive neurological deficit and three with hydrocephalus/increased intracranial pressure. Treatment consisted of radical surgery (four cases), radiosurgery (one case), evacuation of satellite hematoma (two cases), ventriculo-peritoneal shunt (one case) and observation (one case). Operative treatment included the transcallosal approach in two patients, the trigonal in one and occipital supratentorial in one with a cavernoma of the pulvinar.
