The model of ESA is established from an organization perspective, and it is new perspective. The detailed classification system of ESA is developed based on the built ESA model. It is useful to identify the cause chain and root causes of organization causing ESA. The analysis method of ESA is also constructed to guide the investigation of ESA event. A case study is provided to illustrate the concrete application of the method.
