Metatarsophalangeal joint instability of the lesser toe can cause acquired toe deformity. Plantar plate deficiency is the major pathology. Plantar plate repair stabilizes the joint and addresses the primary pathology. The modified technique corrects crossover toe deformity without the need of tendon transfer.
