Decision support systems crucial to the success of the Learning Health System paradigm. Data provenance enables trust in the Learning Health System. We propose provenance templates as a method for implementing data provenance in decision support systems. Example of the provenance-enabled decision support system implemented in the TRANSFoRm project.
