The diagnosis of acral lentiginous melanoma is often challenging, and it is often based on subtle histopathological features. In fact, a more advanced stage at presentation than in other variants of melanoma is one of the features of acral melanoma responsible for the low survival rate associated with this type of malignancy. There are reproducible histopathological criteria for the diagnosis of early acral lentiginous melanoma. With such criteria, the gray zone of “atypical melanocytic proliferation” can be avoided.
