Despite the growing role primary care plays in children’s mental health, there remain difficulties with identification of psychiatric and psychosocial problems and barriers to referral. The child with mental health problems in the emergency department (ED) is particularly prone to be under-identified and the reasons the family views the situation as a crisis are also likely to be misunderstood. Understanding the complex factors that lead to a pediatric mental health crisis presenting in the ED can be enhanced by an appreciation of the broader concept of urgency. This article will describe the spectrum of psychiatric emergencies in youth, present an urgency classification system and triage tool, and give a practical illustration of using urgency data to plan for new services. This broader concept of urgency that includes contextual factors will provide additional guidance to pediatricians faced with mental health problems in the ED.
