Diabetic striatopathy (DS) is an underdiagnosed complication of uncontrolled DM.

DS is characterized by abrupt onset of movement disorder, mainly hemichorea.

The clinical syndrome is accompanied by specific findings on brain imaging.

Treatment includes aggressive control of hyperglycemia with insulin.

Neuroleptic agents can be helpful controlling the neurologic symptoms.
