Summary
Currently, apart from mild, induced hypothermia, no other treatment has shown any ability to decrease the consequences of cerebral anoxia-ischemia due to cardiac arrest. The main role of erythropoietin (Epo) is to support erythropoiesis, but numerous preliminary data also suggested a neuroprotective effect of Epo and its analogues in various experimental models, particularly after neuron damages related to ischemia-reperfusion stress. A recent pilot study showed encouraging results regarding the potentially beneficial effects of high dose epoetine alpha (an analogue of erythropoietin) when administered early after cardiac arrest. In this open label and non-randomized trial, a high proportion of patients survived without significant cerebral disability and without experiencing severe adverse events. To confirm this finding, we designed a randomized, multicentre, simple blind trial in which all participating patients will receive usual treatments and 50 % of them will also receive a high dose of epoetine alpha (an analogue of erythropoietin) in an “add on” fashion. In the interventional group, patients will receive as soon as possible after return of spontaneous circulation a first intravenous dose of Epo (40 000 UI), followed by four supplementary injections (same dose) during the following 48 h. The main end point will be the proportion of patients in each arm who will reach, at day 60, the best level of recovery using the five-level Pittsburgh CPC scale. This clinical trial is funded by the French Ministry of Health and started in November 2009 in 10 centers that associate pre-hospital and ICU teams. Completion of enrolments is planned for the middle of 2012.
