We evaluated myocardial sympathetic function in seven patients with Shy-Drager syndrome using [123I]metaiodobenzylguanidine (MIBG) myocardial scintigraphy. The findings obtained with MIBG, a substance known to be actively taken up by sympathetic nerve terminals, were compared with other indices of autonomic function. The uptake of MIBG into the myocardium was decreased diffusely and markedly in five out of seven patients. In contrast, in all the patients there was a significant reduction of the values of both the low frequency and the high frequency components of the heart rate variability, as demonstrated by power spectrum analysis of ambulatory Holter data.
