Hypertension in the elderly is a risk factor for cardiovascular disease, renal insufficiency and dementia. Previous studies, the HYVET trial, has documented the benefits of blood pressure lowering in the fit elderly patient 80 + years. Caution should be exercised to lower blood pressure in the elderly frail patients with multiple comorbidities and orthostatic reactions. In the SPRINT study, more intensive blood pressure lowering was superior to less intensive treatment for prevention of a composite cardiovascular endpoint and all-cause mortality.
