Toward a taxonomy of the unintentional discharge of firearms in law enforcement

Unintentional discharges typically occured during routine tasks and in contexts with low threat potential. Unintentional discharges involved a variety of firearm manufacturers, models, and trigger functions. Recommendations: (1) assume all firearms are loaded at all times, (2) index, and (3) point the firearm in a safe direction until there is intent to fire. A data-based antecedent-behavior-consequence (A-B-C) taxonomy is presented. A standardized UD reporting form, based on the taxonomy, is offered.
