Pain-related fear was assessed through two groups of different predictability.

Pain predictability affected neural pain processes and altered associative learning.

During aversive learning, unpredictability amplified pain encoding, and modulation.

Pain-related fear does not promote visceral hyperalgesia or central sensitization.
