Résumé
Dans le bilan d’un syndrome lymphoprolifératif chronique B, lorsque la cytologie sanguine et le phénotype ne sont pas typiques d’une LLC-B, la biopsie tissulaire (ganglion, moelle osseuse) voire la splénectomie jouent un rôle important dans le diagnostic du lymphome à petites cellules B. Les auteurs soulignent l’apport des critères histopathologiques et de l’immunohistochimie dans la classification de ces tumeurs. Ils soulignent les difficultés rencontrées selon les différents tissus analysés. Ils insistent également sur, d’une part, l’importance d’un bon conditionnement pré-analytique des prélèvements sous la responsabilité du clinicien, du chirurgien, des techniciens et pathologistes et, d’autre part, sur la nécessité d’une approche diagnostique multidisciplinaire.
Summary
In chronic lymphoproliferative B-cell disorders, when cytology and phenotype are not typical of B-CLL, a biopsy (lymph node, bone marrow) or a splenectomy is often performed for the precise diagnosis of this small B-cell lymphoma. The authors underline the histopathological and immunohistochemical criteria for the classification of these lymphomas. They also underline the pitfalls according to the tissue analysed for the diagnosis. They insist on the importance of a good pre-analytical conditioning of the samples depending on the clinician, surgeon, pathologist, technician and of a multidisciplinary approach for the optimal diagnosis.
