Optic neuritis (ON) in MS is younger-onset, shorter, and shows a better outcome.
ON with serum anti-AQP4-Ab is female-dominant, older-onset, and shows a worse outcome.
ON with serum anti-MOG-Ab is longer with swelling, but shows a better outcome.
Idiopathic ON is much rarer compared to the three demyelinating diseases.
Cerebral MRI and serum autoantibodies should be studied to make a correct diagnosis.
