A traffic accident of a healthy 8-year-old girl resulted in a severe head trauma. After a period of intensive care with indwelling catheter she remained in a stuporous state with diapers and was referred for rehabilitation. Six months later, patient started to notice bladder sensation. A urodynamic investigation(UI) was done and resulted in neuropathic hyperreflex bladder. CIC (4/day) adequate fluid intake and anticholinergics were prescribed. New UI three months later showed no more hyperreflex detrusor. Anticholinergics were stopped, CIC (1/day), fluid balance and /2 h asking for bladder sensation followed by toilet position.
Ten months of urological treatment resulted in 24 h continence, normal uroflow and negligible residue. Simultaneously many other functions partially recovered. Due to the many limitations of the patient, a lot of inventiveness, patience and motivation was asked from all care givers and parents. From this experience an applicable treatment could be deduced.
Incontinence after TBI is often neglected due to the more striking disabilities. In the total process of rehabilitation also continence should be considered.
