Study goal was to support post-detoxification recovery and reduce health care costs. Participants received recovery navigation services with or without incentives. Delivery and uptake of incentives were low; 49% earned any incentives. Health care cost increased from pre- to post-detox and was not reduced with incentives. Barriers to effective incentive implementation are discussed.
