We describe the case of a boy presenting with unilateral bloody discharge. Ultrasound of the breast revealed mammary duct ectasia. After three years, recurrence of the disease in the contralateral breast which is a rare finding. Mammary duct ectasia is a self-limited disease. Occurrence in the contralateral side must raise the suspicion of a systemic disease (endocrine).
