Highly active antiretroviral therapy (HAART) reduces the incidence of opportunistic events and death in patients with HIV infection, but it may be associated with serious complications, like the immune reconstitution inflammatory syndrome (IRIS). This syndrome can include a wide group of infectious, autoimmune and immune-mediated inflammatory diseases. Endocrine autoimmune diseases are rarely diagnosed in patients with IRIS. The authors present a clinical case of a 36-year-old HIV infected Caucasian male who was diagnosed with type 1 diabetes mellitus and with Graves’ disease 16 and 20 months after the beginning of a tolerated and effective HAART regimen, respectively.
