Quantitative muscle ultrasound shows that changes in muscle architecture after stroke are not restricted to the paretic side. The bilateral muscle architecture changes cannot be explained by disuse alone. Echogenicity of the paretic side gastrocnemius muscle correlated moderately with walking speed (inversely) and time since stroke.
