Tacrolimus (FK 506) is a macrolide antibiotic which has immunosuppressive properties 50–100 times more potent than cyclosporine. It is used in prophylaxis of allograft rejection. Tacrolimus blocks the production of interleukin 2, thereby inhibiting T lymphocyte proliferation. Tacrolimus is metabolized solely in the liver and the metabolites are primarily excreted in the bile. Because of its multiple adverse effects (nephrotoxicity, neurotoxicity) monitoring of tacrolimus blood concentration is required. We used a microparticle enzyme immunoassay (MEIA) on the IMX analyser (Abbott). This kit allowing determination of tacrolimus in whole blood, shows acceptable analytic performances: within and between-run CVs of 10 and 12 % respectively, good linearity and sensitivity of 3 μg/l. The tacrolimus test provides a rapid and accurate mean for monitoring tacrolimus therapy.
