The VHA is expanding its continuum of health care services to geriatric and extended care patients and their families and significant others. This expansion involves collaboration and networking with non-VHA health care providers in the community. By increasing the level of awareness among all health care providers of each other's capabilities, opportunities for improving access to, and the delivery of, services will emerge. To learn more about services and programs for geriatric and extended care patients, non-VA health care providers can contact the chiefs of nursing or social work service at their closest VA Medical Center.
