Retrorectal cystic hamartoma (RCH), also known as tailgut cyst, is a rare congenital presacral lesion that must be discussed in women with a long history of perianal disease. We report a case of a 54-year-old woman presenting a retrorectal mass with chronic anal pain. Magnetic resonance imaging shows a multicystic mass in the retrorectal space. Surgical resection by posterior way found a RCH. The main differential diagnoses include others developmental cysts like epidermoid cyst, cystic teratoma and rectal duplication cyst. The main complications are recurrence, chronic inflammation, and malignant transformation. So, complete surgical excision is mandatory.
