High intracranial pulsatility of anterior circulation was related to the lower cognitive status. Longitudinal increment of pulsatility reflects cognitive decline in Alzheimer disease (AD). Longitudinal decrement of mean flow velocity reflects decline in praxis. Serial assessment of transcranial Doppler might be a useful surrogate for AD progression.
