Markers of anhedonia were assessed in recently withdrawn opiate dependent patients.

Patients reported reduced positive affect on Snaith–Hamilton Pleasure Scale (LowPA).

Affect-modulated startle to pleasant images was blunted in patients reporting LowPA.

Patients had reduced activation of prefrontal cortex to food/social reward cues.

Multimodal approach, translational neuroimaging suggest anhedonia in early recovery.
