We investigated whether natural menopause associates with reduced protection from postprandial lipemia in women matched for age and body mass index. In a cross-sectional study, 23 postmenopausal women (mean age 50 ± 1 y, body mass index (BMI) 24.6 ± 2.8 kg/m2) and 21 premenopausal women (mean age 49 ± 1 y, BMI 24.1 ± 2.6 kg/m2) underwent an oral vitamin A fat loading test. Vitamin A is a marker for chylomicrons and chylomicron remnants. All women were normolipidemic, in good health, non-smokers and did not use any medication. Postprandial responses, measured at hourly intervals, are expressed as area under the curve (AUC) from zero (baseline) to eight hours after the test meal. In the postprandial phase significant differences were found in total postprandial triglyceride AUC (+36 % , P = 0.024) and vitamin A AUC (+50 % , P = 0.001). Fasting triglycerides (TG) were not significantly elevated in postmenopausal women (1.14 ± 0.57 mmol/L) when compared to premenopausal women (0.88 ± 0.33 mmol/L). To assess possible confounding effects of fasting TG, 13 postmenopausal women were matched with 19 premenopausal women. Although fasting TG levels were identical (0.72 ± 0.22 vs 0.73 ± 0.22 mmol/L), postprandial vitamin A AUC and incremental postprandial TG AUC (i.e. after correction for baseline TG) were 38 % (P = 0.006) and 42 % (P = 0.023) higher in postmenopausal women, respectively.
