Bilateral maxillary defect remains a challenging task for surgeons. Composite three-dimensional maxillary wounds often require double free flaps for satisfactory outcome. Chimeric free flap has recently been evolving as a good alternative option. From our experience of harvesting the submental flap, we noticed that at least one perforator from the submental artery coursed towards the inferior border of the mandible, which is potentially a new osteocutaneous chimeric flap. We reported one case of composite defect created after maxillectomy for osteosarcoma of the anterior upper jaw, which was successfully reconstructed by a pedicled submental osteocutaneous chimeric mandible flap. To our knowledge, this innovative design of osseous flap in chimeric style based on the submental vessels has not been described.
