We assessed rates of good and poor outcomes at 90 days after tPA therapy.

Outcomes were good (mRS score 0–1) and poor (4–6) outcomes and death.

DWI lesion volumes at 7 days could predict good and poor outcomes at 90 days.

The cut-off values for good and poor outcomes and death were 15 mL, 30 mL and 100 mL.

We can predict outcome at 90 days by measuring subacute DWI lesion volume.
